2024-25 Athletic Registration Forms

Skyline Athletic Registration Forms Must be completed online. This is a new registration site and everyone must register using the new site. Parents/Guardians can register and sign for their student in the sections that require student signatures. There is no longer a parent account and separate student account.


Insurance & Pay to Participate Fee Per AAPS policy, these fees must be paid online or call the Rec & Ed office, 734-994-2300 for help, as part of the registration process and before any student will be cleared for participation in athletics, including tryouts. Regardless of what team you are participating with, Skyline or a co-op team with Huron or Pioneer, you will pay the fees for Skyline.


AAPS Waiver Form Only used by those who no longer have their Chartwell’s letter stating Free/Reduced Lunch status.  Must be turned in to athletic office who will verify status with Chartwell’s and then process the waiver code request for PTP fees. The Chartwell’s letter is the fastest way to obtain a waiver code but please do not request another letter as one will not be provided.  Just fill out the waiver form and turn it into the athletic office. Do NOT pay any fees until your waiver status has been verified. The 23-24 status for students will be used as temporary status for the fall season only. Anyone wanting to participate in the winter or spring seasons and needing a reduced fee code, must fill out a new application on the Food Service website before the start of the season. Thank you.


MHSAA New Student/Transfer Form Must be filled out by any student in 10th, 11th or 12th grade new to Skyline, any 9th grader new to Skyline only after the school year has begun.

A blank copy of the MHSAA Physical Exam Form can be obtained on the registration site(Must be uploaded onto the registration site once completed by physician, original copy is to be keep with family). A full copy of the form is not needed to register, just a copy of the physical exam from the medical examiner’s office is fine as the rest of the MHSAA form will be completed online as part of the registration.

*Physical Examination Requirements for Playing a Sport at Skyline
Student’s will NOT be allowed to practice with or try-out for a Skyline team unless a current physical form is on file in the Athletic Office prior to the first practice. NO EXCEPTIONS.

The physical examination must:

  • Be signed by a physician (M.D. or D.O.), Physician’s Assistant, or Nurse Practitioner. Forms signed by chiropractors will not be accepted!
  • Be dated on or after April 15 of the previous school year.

NOTE: The penalty to the school for allowing a student to participate in a contest without having a current signed physical card on file is a forfeit.  However, of far greater significance is the risk of physical activity without clearance from a physician.

SKYLINE SPORTS REGISTRATION STEPS

STEP ONE: Pay-to-Participate and Insurance Fees must be paid through Ann Arbor Community Education and Recreation online with credit card (See link above or on the Skyline Athletic Registration site). NOTE: Club sports will pay the insurance fee only.

Per Board of Education Policy titled Requirements, Regulations & Tryouts(6420.R.07):  Section 5 Procedures:

5.1    Registration: Before trying out or practicing with any team(s), student athletes must have the following on file in the Athletic Department office:

5.1.4  pay to participate fee and letter of understanding (refundable only if athlete is cut from the team)

Please note: The sports physical will not be approved on Arbiter Student Registration until payment is made. Payment is noted in the physical exam section.

If you believe you are eligible for a refund, please contact the Athletic Office, not Rec & Ed.  NO REFUNDS for Pay-to-Participate will be given after June 1st of the current school year.

STEP TWO:  The Athletic Department at Ann Arbor Skyline High School now hosts their Pre-Participation Athletic Forms online with Arbiter Student Registration. This digital platform will allow you, and your student(s), to complete and access athletic forms via computer, tablet, or mobile phone. It is FERPA compliant. A link to the privacy policy is located at the bottom of the Arbiter Student Registration site.

Important: Please make sure you use the student Legal Name from school registration, DOB, high school graduation year and school(please use the school the student attends if not Skyline).

  1. Go to Arbiter Student Registration and click on the link in the Programs section under the photos.
  2. Complete Athletic Forms/Uploads as Advised.

Athletes cannot participate in sports until digital forms are complete and approved and all district fees are paid. Forms will be valid for the entire school year for which they are filled out. Completing and having your Pre-Participation Registration Forms accepted by your school does NOT guarantee you have made the team.
